參加者聲明及保證
本人現參加「粉紅瑜伽2025」（下稱：活動），謹此聲明並保證，本人會遵守所有有關的活動條款和細則和香港遺傳性乳癌家族資料庫（下稱：主辦單位）的安排。倘於活動期間不論任何原因發生意外，包括死亡、受傷或財物損失，本人將自行承擔一切責任及損失。主辦單位、協辦機構、各贊助機構及支援機構毋須對此作出賠償或負上任何法律責任。若本人在活動期間發生事故而導致主辦單位支付額外開支，本人願意對主辦單位彌償有關額外開支。
條款及細則
- 本活動由香港遺傳性乳癌家族資料庫主辦。
- 所有報名必須於2024年12月31日 11:59pm (香港時間) 之前完成。主辦單位不接受逾期報名。
- 若申請者提供錯誤資料、沒有或逾期繳交報名表及費用，或不依從正確報名程序報名，主辦單位有權拒絕接受有關報名之權利。報名一經接納，費用恕不發還。已提交的報名或重複報名，均不設退款、退件、轉名安排及取消服務。
- 主辦單位保留限制及拒絕接受報名的權利。
- 主辦單位保留聯絡申請者之權利，以便索取更多有關其報名資料之事宜。
- 主辦單位有權隨時停止報名，而事先不作任何通知。
- 如遇惡劣天氣或天文台於活動當日早上6：00仍發出黃、紅、黑雨警告，三號或以上颱風訊號，活動將會取消。已繳交的報名費和捐款，將作捐款論及不會獲退還。請留意本網頁及主辦單位Facebook 專頁: https://www.facebook.com/asiabreastregistry/的最新公布。
- 注意安全是參加活動的首要原則，活動期間請遵從主辦單位的指示。
- 主辦單位將為是次活動購買第三者責任保險，參加者可自行考慮購買個人意外保險。主辦單位不承擔參加者的個人保險責任。
- 參加者應自行評估自身健康狀況，包括身體狀態、心理狀態和任何個人病史，以確定是否適宜參加活動。主辦單位不對參加者的健康狀況進行評估，並不承擔任何由參加活動對參加者健康狀況可能產生的負面影響或後果責任。有需要時，參加者在參加活動之前諮詢醫生或專業人士的意見，並根據個人情況做出明智的決策。
- 本活動不提供行李寄存區。請自行保管貴重物品，主辦單位對於場地内任何損失或損壞不負責任。
- 主辦單位有權使用參加者的照片、影像、錄像和其他相關素材，包括但不限於在官方網站、社交媒體平台、報紙、雜誌、宣傳物料、廣告和其他宣傳渠道上使用作宣傳和推廣活動。
- 活動安排如有更改，將以主辦單位公佈為準，恕不另行通知。如有任何爭議，主辦單位持最終決定權。
收集個人資料聲明
- 參加者所提供的資料 (包括姓名、電話及電郵地址等等)將可能日後用於香港遺傳性乳癌家族資料庫募捐或活動推廣用途。所提供的資料可能被轉移至第三者為達成上述用途。你有權要求存取和更正資料。如閣下不欲香港遺傳性乳癌家族資料庫將有關個人資料用於上述用途，請電郵至[email protected]，標題寫上”Unsubscribe”或 ”取消訂閱”。
- 除非閣下事先同意或根據香港的法院命令、指令或指引要求，香港遺傳性乳癌家族資料庫除上述用途，不會向第三者披露閣下個人資料。
Disclaimer and indemnity
I wish to participate in the “Pink Yoga 2025” (the Event). I confirm and agree that I shall comply with all relevant rules, regulations, arrangements, and /or decisions made by Hong Kong Hereditary Breast Cancer Family Registry (the Organizer). I take part in this event entirely at my own risks and that I shall not hold the Organizer, the co-organizer, any sponsors and /or supporting organizations responsible for any accident of whatever kind for any reason, resulting in death or injury, and /or for any damage to, loss or destruction of personal properties during the course of the Event. I shall also agree to indemnify and reimburse the Organizer with respect to any additional expenses or costs incurred by the Organizer arising from or in connection with my participation in the Event.
Terms and Conditions
- This event is organized by the Hong Kong Hereditary Breast Cancer Family Registry.
- Registration must be completed on or before 11:59pm (HKT), 31st December, 2024. Late application will not be accepted.
- If the participants provide incorrect information, fail to submit application form, pay for the fee on time or fail to follow the enrollment procedure, the Organizer shall have the right to reject their application. Once the application is submitted, no request for refund, return for duplicated fees, transfer and cancellation of the enrollment will be entertained.
- The Organizer reserves the right to limit and refuse any enrollment.
- The Organizer reserves the right to contact participants for additional information regarding their enrollment.
- The Organizer reserves the right to close the enrollment at any time without prior notification.
- If there is adverse weather or Amber/Red/Black Rainstorm Signal, Tropical Cyclone Warning Signal No. 3 or above being hoisted and not lowered at 6:00am on the event date, the event will be cancelled. The enrollment fee and sponsorship will be treated as a donation and not be refunded. - Safety is a top priority for participating in the event. Participants must always follow the instructions of the staff of the Organizer.
- The Organizer will provide the third-party liability insurance coverage for the event. Participants are advised to consider purchasing personal accident insurance to cover any potential accidents or injuries. The Organizer is not responsible for participants’ personal insurance coverage.
- Participants should assess their own health conditions, including physical and mental states, as well as any personal medical history, to determine if they are suitable and fit to participate in the Event. The Organizer does not evaluate participants’ health conditions and is not responsible for any adverse effects or consequences on participants’ health resulting from participating in the Event. If necessary, participants may consider to consult with a doctor or healthcare professional and make informed decisions based on their individual circumstances before participating.
- There is no baggage storage area provided at the event. Please take care of your valuables, as the organizer is not responsible for any loss or damage within the venue.
- The Organizer has the right to use participants’ photos, videos, recordings, and other related materials for promotional and marketing purposes, including but not limited to official websites, social media platforms, newspapers, magazines, promotional materials, advertisements, and other promotional channels.
- The Organizer will make announcements on the changes in the arrangement or regulations of the event, if any, without prior notice. In case of any dispute, the decision of the Organizer will be final.
PERSONAL INFORMATION COLLECTION STATEMENT
- The information you provided (including name, telephone number, email address and etc) will be used by Hong Kong Hereditary Breast Cancer Family Registry for fund-raising and event promotion. The data may be provided to other persons who may facilitate the Organizer to carry out the purposes mentioned above. You have the right to request access to and correction of the data. If you do not wish your personal information be used for the abovementioned purposes, please send email to [email protected] with the title of “Unsubscribe” or ”取消訂閱”.
- Hong Kong Hereditary Breast Cancer Family Registry will not disclose your personal information to third parties other than for the above purpose without your permission, unless compelled to do so by any judicial request, requirement or court order.
